You will be a key contact for patients, their families, and other healthcare professionals, providing support and guidance throughout the patient’s care journey.To excel in this role, you must possess the following qualifications: a Bachelor’s degree in social work, psychology, health care administration, or a related field; previous experience in a hospital or healthcare setting; excellent communication and organizational skills; and a strong understanding of the healthcare system. Responsibilities:

  1. Provide support and guidance to patients, their families, and healthcare professionals throughout the patient care journey.
  2. Coordinate patient services to ensure that they receive the best possible treatment and care.
  3. Serve as a key contact to answer questions and address any concerns.
  4. Maintain an up-to-date knowledge of the healthcare system.
  5. Create and implement effective plans for patient care.
  6. Monitor patient progress and ensure that medical and care plans are properly followed.
  7. Collaborate with other healthcare professionals to coordinate care.
  8. Document patient interactions and treatment plans.
  9. Ensure compliance with all relevant laws and regulations.
  10. Provide regular updates to patients and their families on their progress.
