
Wound Care Coordinator

- Conduct thorough assessments of patients' wounds and develop individualized treatment plans.
- Collaborate with physicians and other healthcare professionals to determine the best course of treatment.
- Monitor and evaluate patients' progress and make necessary adjustments to their treatment plans.
- Provide education and support to patients and their families on wound care management, prevention, and self-care techniques.
- Maintain accurate and detailed documentation of all wound care treatments and interventions.
- Keep up-to-date with the latest wound care techniques, products, and research to ensure patients receive the best possible care.
- Oversee the ordering and inventory management of wound care supplies.
- Communicate regularly with interdisciplinary team members to ensure coordinated and comprehensive care for patients.
- Participate in quality improvement initiatives and ensure compliance with all regulatory standards related to wound care.
- Serve as a resource for staff and provide training and education on wound care best practices.
- Act as an advocate for patients, ensuring their needs and concerns are addressed in a timely and compassionate manner.
- Maintain confidentiality and adhere to ethical standards in handling patient information.
- Contribute to a positive and professional work environment by demonstrating teamwork, respect, and a commitment to excellence.
- Attend meetings, conferences, and workshops to enhance professional development and stay current on industry trends and advancements.
- Other duties as assigned by the management team.
Registered Nurse (Rn) License In The State Of Employment.
Minimum Of 2 Years Experience In Wound Care, Preferably In An Acute Care Or Rehabilitation Setting.
Certification In Wound Care, Such As Wound Care Certified (Wcc) Or Certified Wound Specialist (Cws).
Strong Clinical Assessment And Critical Thinking Skills, With The Ability To Develop Individualized Treatment Plans For Complex Wounds.
Excellent Communication And Leadership Skills, With The Ability To Educate And Train Other Healthcare Professionals On Wound Care Best Practices.
