3. Assess patients' needs and goals to determine the most appropriate rehabilitation program and services.

5. Collaborate with the interdisciplinary team to develop customized treatment plans for patients.
6. Monitor and track patient progress and communicate updates to the treatment team and referral sources.
7. Provide support and guidance to patients and their families throughout the rehabilitation process.
8. Conduct on-site visits and presentations to potential referral sources, promoting Encompass Health's services and capabilities.
9. Stay up-to-date on industry trends and changes in rehabilitation services, and share this information with the team.
10. Participate in marketing and networking events to increase visibility and promote Encompass Health's services.
11. Maintain accurate records and documentation of patient interactions and referrals.
12. Adhere to all regulatory and compliance standards related to patient care and confidentiality.
13. Collaborate with insurance providers to ensure timely authorization and payment for services.
14. Demonstrate a commitment to providing exceptional customer service and maintaining a positive reputation for Encompass Health.
15. Attend regular team meetings and trainings to continuously improve skills and knowledge related to the role.
