Rehab Liaison

In this role, you will serve as a liaison between our facility and external referral sources to facilitate the admission process for patients in need of rehabilitation services. 1. Serve as the primary point of contact for external referral sources, including hospitals, physicians, and insurance companies.
2. Conduct outreach to potential referral sources, including in-person visits, phone calls, and email communication.
3. Educate referral sources on the services and benefits offered by Encompass Health's rehabilitation programs.
4. Evaluate patient referrals to determine eligibility for admission to Encompass Health.
5. Coordinate with internal admissions team to facilitate the admission process for referred patients.
6. Maintain accurate and up-to-date records of referral sources and patient admissions.
7. Collaborate with the interdisciplinary team at Encompass Health to ensure a smooth transition for patients from referral source to our facility.
8. Provide exceptional customer service to referral sources, patients, and their families.
9. Participate in team meetings and trainings to stay informed about Encompass Health's services and processes.
10. Act as a brand ambassador for Encompass Health, promoting our mission and values to referral sources and the community.
11. Monitor and report on referral trends and market competition to inform business development strategies.
12. Maintain confidentiality of patient information and comply with HIPAA regulations.
13. Adhere to all Encompass Health policies and procedures.
14. Continuously improve and expand knowledge of rehabilitation services and healthcare industry developments.
15. Represent Encompass Health in a professional and ethical manner at all times.
