Rehab Liaison

1. Build and maintain relationships with healthcare professionals, including physicians, nurses, and case managers, to promote Encompass Health and its services.
2. Develop relationships with community organizations and attend networking events to increase awareness of Encompass Health and its services.
3. Collaborate with case managers and discharge planners to identify potential patients who may benefit from Encompass Health's services.
4. Conduct on-site assessments of potential patients and educate them on the services and benefits of Encompass Health.
5. Serve as a knowledgeable resource for patients, families, and external referral sources regarding the services provided by Encompass Health.
6. Communicate with patients and their families about the admissions process and answer any questions or concerns they may have.
7. Work closely with the admissions team to ensure a smooth transition for patients from referral sources to Encompass Health.
8. Monitor referral trends and analyze data to identify areas for improvement and develop strategies to increase referrals.
9. Maintain accurate and up-to-date records of all referrals and communication with external sources.
10. Attend regular team meetings and provide input on ways to improve patient care and the referral process.
11. Stay up-to-date on industry trends and changes, as well as Encompass Health's services and programs, to effectively communicate with referral sources.
12. Represent Encompass Health at conferences, seminars, and other events to promote the organization and its services.
13. Adhere to all legal and ethical standards and maintain patient confidentiality at all times.
14. Collaborate with the marketing team to develop and implement marketing strategies to increase referrals and promote Encompass Health's services.
15. Continuously strive to meet and exceed referral goals and contribute to the overall success of the organization.
