- Serve as a representative of the rehabilitation facility to the community, promoting our services and building relationships with potential referral sources.
- Utilize clinical knowledge to educate and inform potential patients, families, and referral sources about our rehabilitation programs.
- Conduct assessments of potential patients and make recommendations for appropriate levels of care.
- Collaborate with the interdisciplinary team to develop individualized treatment plans for patients.
- Maintain accurate and up-to-date records of patient interactions, referrals, and admissions.
- Coordinate and facilitate smooth transitions for patients from acute care settings to our rehabilitation facility.
- Communicate with insurance companies and other payors to ensure proper authorization and coverage for patients.
- Act as a liaison between patients, families, and the rehabilitation team to ensure a seamless and positive experience for all involved.
- Continuously monitor and evaluate the success of our referral and admission processes, making adjustments as needed.
- Participate in community events and networking opportunities to promote the facility and increase referrals.
- Stay current on industry trends and changes in healthcare regulations, and incorporate this knowledge into daily responsibilities.
- Uphold the mission, values, and standards of Encompass Health in all interactions with patients, families, and the community.
Bachelor's Degree In Healthcare-Related Field: A Degree In A Healthcare-Related Field Such As Nursing, Physical Therapy, Or Occupational Therapy Is Essential For A Rehab Liaison Position At Encompass Health. This Ensures A Strong Understanding Of Healthcare Practices And Terminology.
Experience In Rehabilitation Or Acute Care Setting: Candidates Should Have Prior Experience Working In A Rehabilitation Or Acute Care Setting. This Provides A Foundation Of Knowledge And Skills Necessary For The Job, As Well As Familiarity With The Patient Population.
