- Dispensing Medications: The Pharmacist will be responsible for accurately dispensing medications to patients based on physician orders and ensuring that all medications are properly labeled and stored.
- Medication Management: The Pharmacist will be responsible for conducting medication therapy reviews and monitoring patient medication regimens to ensure safe and effective use of medications.
- Patient Counseling: The Pharmacist will provide patient counseling and education on medication usage, potential side effects, and proper storage and disposal of medications.
- Collaborate with Healthcare Team: The Pharmacist will work closely with physicians, nurses, and other healthcare professionals to ensure appropriate medication therapy for patients and to address any medication-related concerns or issues.
- Maintain Regulatory Compliance: The Pharmacist will adhere to all state and federal regulations regarding the handling and dispensing of medications and maintain accurate records of all dispensed medications.
- Medication Procurement: The Pharmacist will be responsible for ordering and managing medication inventory, ensuring an adequate supply of medications for patient care.
- Quality Assurance: The Pharmacist will participate in quality assurance activities, such as medication error reporting and medication use evaluations, to identify and address opportunities for improvement in medication management.
- Professional Development: The Pharmacist will stay current with new medications, dosage forms, and treatment protocols through continuing education and professional development opportunities.
- Adhere to Best Practices: The Pharmacist will follow evidence-based medication practices and contribute to the development and implementation of medication-related policies and procedures.
- Patient Advocacy: The Pharmacist will advocate for patients’ medication needs, including identifying and addressing barriers to medication access and affordability.
- Maintain a Safe Environment: The Pharmacist will maintain a safe working environment by adhering to infection control and medication safety protocols.
- Interpersonal Communication: The Pharmacist will communicate effectively and professionally with patients, caregivers, and healthcare team members to promote positive patient outcomes and a high level of patient satisfaction.
Bachelor's Or Doctor Of Pharmacy Degree From An Accredited Institution
Current And Active State Pharmacy License
Minimum Of 2 Years Of Experience In A Clinical Or Hospital Pharmacy Setting
