  1. Coordinate and oversee the patient assessment process, ensuring that all assessments are completed accurately and in a timely manner.
  2. Collaborate with healthcare team members to gather and review patient information, ensuring that all assessments are comprehensive and reflect the patient’s current condition.
  3. Monitor and track patient assessment data, identifying any discrepancies or areas for improvement.
  4. Develop and implement processes to improve the efficiency and accuracy of patient assessments.
  5. Stay up-to-date on industry standards and regulations related to patient assessments, and ensure that all assessments are in compliance.
  6. Train and educate healthcare staff on proper assessment procedures and protocols.
  7. Work closely with other departments, such as nursing and therapy, to ensure a coordinated and holistic approach to patient care.
  8. Communicate with patients and their families to explain assessment procedures and answer any questions or concerns.
  9. Maintain accurate and organized records of patient assessments and related documentation.
  10. Collaborate with insurance companies and other external entities to ensure proper reimbursement for services provided.
  11. Participate in quality improvement initiatives and provide recommendations for enhancing the patient assessment process.
  12. Serve as a patient advocate, ensuring that all assessments are conducted with empathy, respect, and compassion.
  13. Continuously evaluate and assess the effectiveness of the patient assessment process, making adjustments and improvements as needed.
  14. Adhere to all ethical and legal standards related to patient privacy and confidentiality.
  15. Act as a resource for healthcare staff, providing guidance and support on patient assessment protocols and procedures.

Job Qualifications
  • Strong Analytical And Problem-Solving Skills

  • Bachelor's Degree In Nursing Or Healthcare Administration

  • Minimum Of 3-5 Years Of Experience In Healthcare Quality Improvement And/Or Patient Assessment Standards Coordination

  • Knowledge Of Relevant Healthcare Regulations And Accreditation Standards (E.g. Joint Commission, Cms)

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Multidisciplinary Teams
