  1. Develop and implement personalized nutrition plans for patients based on their individual needs and medical conditions.
  2. Conduct regular assessments and evaluations to monitor patients' progress and make necessary adjustments to their nutrition plans.
  3. Provide education and counseling to patients and their families on healthy eating habits, lifestyle changes, and disease management.
  4. Collaborate with interdisciplinary team members, including physicians, nurses, and therapists, to ensure coordinated and comprehensive care for patients.
  5. Stay up-to-date on the latest research and developments in the field of nutrition and incorporate new knowledge into practice.
  6. Adhere to all relevant healthcare regulations and guidelines, including HIPAA and state licensing requirements.
  7. Maintain accurate and thorough patient records, documenting all interventions, progress, and outcomes.
  8. Participate in team meetings and contribute to the development of patient care plans.
  9. Provide support and guidance to patients transitioning to home care, including meal planning and grocery shopping assistance.
  10. Serve as a resource for other healthcare professionals, providing guidance and education on nutrition-related topics.
  11. Advocate for the role of nutrition in overall health and wellness, both within the organization and in the community.
  12. Continuously evaluate and improve the effectiveness of nutrition services, implementing changes as needed to ensure optimal patient outcomes.
  13. Maintain a professional and compassionate approach when interacting with patients, their families, and other healthcare professionals.
  14. Adhere to ethical standards and maintain confidentiality in all aspects of the job.
  15. Attend conferences, workshops, and other educational opportunities to enhance knowledge and skills in the field of nutrition.

Job Qualifications
  • Knowledge Of Medical Nutrition Therapy And Ability To Develop Individualized Meal Plans For Patients With Complex Medical Conditions.

  • Bachelor's Or Master's Degree In Nutrition Or Dietetics From An Accredited Institution.

  • Registered Dietitian (Rd) Credential From The Commission On Dietetic Registration (Cdr).

  • Minimum Of 2-3 Years Of Experience In Clinical Nutrition, Preferably In A Rehabilitation Or Acute Care Setting.

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Team Members And Educate Patients On Nutrition And Lifestyle Changes.
