1. Provide comprehensive case management services to patients, including assessment, planning, referral, and coordination of care.
2. Develop and implement individualized care plans in collaboration with the patient, their family, and healthcare team.
3. Serve as the primary point of contact for patients and their families, addressing any concerns or questions they may have throughout the care process.
4. Maintain accurate and thorough documentation of patient progress and treatment plans.
5. Collaborate with healthcare team members to ensure seamless and coordinated care for patients.
6. Advocate for patients' needs and rights, and ensure they receive the highest quality of care.
7. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
8. Educate patients and their families about their medical condition, treatment options, and available resources.
9. Coordinate and facilitate discharge planning and transition of care for patients.
10. Participate in interdisciplinary team meetings to discuss patient progress and make recommendations for care.
11. Stay up-to-date on current best practices and standards in case management and healthcare.
12. Maintain confidentiality and adhere to all HIPAA regulations in all interactions with patients and their families.
13. Serve as a positive role model and representative of Encompass Health values and mission.
14. Collaborate with community resources to provide patients with additional support and resources as needed.
15. Adhere to all Encompass Health policies and procedures and maintain a safe and professional work environment.
