  1. Develop and implement individualized care plans for patients, based on their specific needs and goals for recovery.
  2. Collaborate with a multidisciplinary team, including physicians, nurses, therapists, and social workers, to ensure the highest level of care is provided to patients.
  3. Advocate for the best interests of patients, ensuring that their physical, emotional, and social needs are met.
  4. Communicate effectively with patients, their families, and other healthcare professionals to provide updates on patient progress and address any concerns or issues.
  5. Monitor and assess patient progress, adjusting care plans as needed to ensure optimal outcomes.
  6. Provide education and support to patients and their families, helping them understand their conditions, treatment plans, and available resources.
  7. Maintain accurate and detailed records of patient care and progress.
  8. Collaborate with insurance companies, discharge planners, and other agencies to coordinate and facilitate the discharge process for patients.
  9. Stay up-to-date on the latest developments and advancements in healthcare and social services to provide the best possible care to patients.
  10. Adhere to all regulatory and compliance standards and guidelines.
  11. Demonstrate compassion, empathy, and professionalism in all interactions with patients and their families.
  12. Work effectively in a fast-paced and dynamic environment, managing multiple tasks and priorities.
  13. Conduct initial assessments and screenings to identify patient needs and determine appropriate levels of care.
  14. Participate in regular team meetings and case conferences to discuss patient progress and develop effective care plans.
  15. Provide crisis intervention and support to patients and their families as needed.

Job Qualifications
