  1. Coordinate and manage the care of patients to ensure they receive the best possible treatment and support.
  2. Conduct thorough assessments of patients' needs and develop personalized care plans.
  3. Work closely with physicians, therapists, and other healthcare professionals to create an integrated treatment approach for patients.
  4. Monitor and evaluate patients' progress and make adjustments to treatment plans as needed.
  5. Communicate effectively with patients and their families to provide education, support, and guidance throughout the recovery process.
  6. Collaborate with insurance providers and other agencies to obtain necessary authorizations and resources for patients' care.
  7. Maintain accurate and up-to-date documentation of patient care and progress.
  8. Advocate for patients' needs and ensure they are receiving appropriate and timely care.
  9. Identify and address any potential barriers to patients' recovery and independence.
  10. Participate in interdisciplinary team meetings to discuss patient progress and coordinate care.
  11. Stay current on industry trends and best practices in case management.
  12. Adhere to all company policies, procedures, and regulations.
  13. Foster a positive and supportive work environment for colleagues and patients.
  14. Provide exceptional customer service to patients and their families.
  15. Continuously strive to improve the quality of care and services provided to patients.

Job Qualifications
