  1. Coordinate patient care: The primary responsibility of the Case Manager is to coordinate the care of patients, ensuring that all medical and emotional needs are being met.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Nursing, Or A Related Field.

  • Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting.

  • Strong Understanding Of Healthcare Systems And Insurance Processes.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Healthcare Professionals.

  • Knowledge Of Local Community Resources And Ability To Connect Patients With Appropriate Services.
