1. Lead and manage a team of case managers to ensure high-quality and compassionate care for patients and their families.
2. Collaborate with healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans.
3. Ensure effective coordination of services, including home health, hospice, rehabilitation, and other resources, to meet the needs of patients.
4. Advocate for patients and their families to ensure their needs and preferences are considered in the care planning process.
5. Develop and maintain policies and procedures related to case management to ensure compliance with healthcare regulations and standards.
6. Monitor and evaluate the performance of the case management team and provide coaching and development opportunities as needed.
7. Maintain accurate and up-to-date patient records and documentation in accordance with HIPAA regulations.
8. Identify opportunities for improvement in case management processes and implement solutions.
9. Collaborate with other departments and staff to ensure a coordinated approach to patient care.
10. Communicate effectively with patients, families, and healthcare team members to provide updates, address concerns, and promote a positive care experience.
11. Stay current on industry trends and best practices in case management to inform decision-making and ensure quality care.
12. Uphold Encompass Health’s mission, vision, and values in all interactions and decisions.
13. Participate in meetings, committees, and other activities to promote effective communication and collaboration within the organization.
