1. Greet and welcome patients and their families upon arrival to our rehabilitation facilities.
2. Educate patients and their families on the admission process and answer any questions or concerns they may have.
3. Work closely with medical staff and case managers to ensure a smooth and timely admission process for each patient.
4. Build and maintain strong relationships with referring hospitals, physicians, and other healthcare professionals.
5. Conduct thorough assessments of potential patients to determine their eligibility for admission.
6. Collaborate with the admissions team to develop individualized care plans for each patient.
7. Maintain accurate and up-to-date records of patient information and admissions processes.
8. Communicate effectively with patients and their families to provide updates on the status of their admission.
9. Provide emotional support and guidance to patients and their families during the admissions process.
10. Continuously strive to improve the admissions process and provide excellent customer service to patients and their families.
11. Participate in marketing and outreach activities to promote the services of our rehabilitation facilities.
12. Stay updated on industry trends and changes in regulations related to admissions.
13. Adhere to all patient confidentiality and privacy policies.
14. Collaborate with the discharge planning team to ensure a seamless transition for patients from admission to discharge.
15. Attend regular staff meetings and training sessions to enhance job knowledge and skills.
