  1. Provide psychosocial support to patients and their families affected by infectious diseases.
  2. Advocate for patients' rights and needs within the healthcare system.
  3. Collaborate with the interdisciplinary team to develop comprehensive care plans for patients.
  4. Conduct psychosocial assessments and provide appropriate interventions.
  5. Facilitate support groups and educational programs for patients and families.
  6. Connect patients and families with community resources and support services.
  7. Document all patient interactions according to department and institutional guidelines.
  8. Maintain a working knowledge of current social work principles and practices.
  9. Participate in team meetings and case conferences to discuss patient care and progress.
  10. Serve as a liaison between patients, families, and healthcare providers.
  11. Continuously assess and address the social and emotional needs of patients and families.
  12. Collaborate with outside agencies and organizations to coordinate care for patients.
  13. Provide crisis intervention and support during difficult situations.
  14. Maintain confidentiality and adhere to ethical standards in all interactions with patients and families.
  15. Demonstrate a commitment to ongoing professional development and education in the field of social work.

Job Qualifications
  • Bachelor's Degree In Social Work From An Accredited Program

  • Previous Experience Working With Vulnerable Populations, Specifically In The Field Of Infectious Diseases

  • Knowledge Of Relevant Laws And Regulations Related To Healthcare And Social Work, Such As Hipaa And Patient Confidentiality

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Diverse Individuals And Teams

  • Ability To Handle Complex And Sensitive Situations With Empathy And Professionalism, While Adhering To Ethical Standards Of The Social Work Profession.
