  14. Maintain patient confidentiality at all times.

Job Qualifications
  • Minimum Of 2 Years Of Experience In Social Work Or Case Management In A Healthcare Setting.

  • Bachelor's Or Master's Degree In Social Work Or Related Field From An Accredited Institution.

  • Knowledge Of State And Federal Regulations Related To Social Work And Case Management, Including Hipaa And Medicare/Medicaid.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Patients.

  • Strong Assessment, Problem-Solving, And Advocacy Skills, With The Ability To Develop Effective Care Plans And Connect Patients With Appropriate Resources And Services.
