- Adhere to all ethical and legal standards in patient care.
- Participate in team meetings and case conferences to discuss patient care.
- Communicate effectively with patients, families, and healthcare team members.
- Handle difficult and sensitive situations with empathy and professionalism.
- Maintain patient confidentiality at all times.

Minimum Of 2 Years Of Experience In Social Work Or Case Management In A Healthcare Setting.
Bachelor's Or Master's Degree In Social Work Or Related Field From An Accredited Institution.
Knowledge Of State And Federal Regulations Related To Social Work And Case Management, Including Hipaa And Medicare/Medicaid.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Patients.
Strong Assessment, Problem-Solving, And Advocacy Skills, With The Ability To Develop Effective Care Plans And Connect Patients With Appropriate Resources And Services.
