As a Transplant Coordinator, you will play a crucial role in coordinating and managing the care of our transplant patients.   1. Coordinate and manage the care of transplant patients throughout their entire transplant journey, from pre-transplant evaluation to post-transplant follow-up.
  2. Collaborate with multidisciplinary teams to develop and implement individualized care plans for each patient.
  3. Serve as the primary point of contact for patients, families, and healthcare providers involved in the transplant process.
  4. Conduct thorough assessments of patients' physical, emotional, and psychosocial needs and provide appropriate support and resources.
  5. Educate patients and families on transplant procedures, medications, and post-transplant care to ensure understanding and compliance.
  6. Monitor and evaluate patients for potential complications and provide timely interventions as needed.
  7. Maintain accurate and up-to-date documentation of patient care and progress.
  8. Facilitate communication and coordination between different departments and healthcare professionals to ensure seamless care for patients.
  9. Assist with the coordination of donor and recipient evaluations and the transplant surgery process.
  10. Participate in quality improvement initiatives to enhance the transplant program and ensure best practices are being followed.
  11. Stay current on advancements in transplant medicine and participate in continuing education opportunities.
  12. Provide emotional support and counseling to patients and families throughout the transplant process.
  13. Adhere to all legal, ethical, and professional standards in the delivery of patient care.
  14. Maintain confidentiality and protect patient information in accordance with HIPAA regulations.
  15. Collaborate with insurance providers to ensure appropriate coverage and reimbursement for transplant services.
  16. Serve as an advocate for patients, ensuring their needs and concerns are addressed and their rights are respected.
  17. Participate in on-call rotation for after-hours patient needs and emergencies.

Job Qualifications
  • Current Rn Licensure In The State Of Georgia

  • Minimum Of 2 Years Of Experience In Transplant Nursing, Preferably In A Coordinator Role

  • Bls And Acls Certifications

  • Knowledge And Understanding Of Transplant Regulations And Standards Of Care

  • Strong Communication And Organizational Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.
