  1. Implement and manage patient care plans in collaboration with the care team and according to established standards and protocols.
  2. Serve as a liaison between patients, families, and healthcare professionals to ensure effective communication and coordination of care.
  3. Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs and develop individualized care plans.
  4. Monitor patient progress and provide support and education to patients and their families.
  5. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement evidence-based interventions and treatments.
  6. Document all patient interactions and interventions accurately and timely in electronic medical records.
  7. Maintain knowledge of current healthcare regulations, policies, and procedures to ensure compliance.
  8. Participate in quality improvement initiatives and contribute to the development of best practices for patient care.
  9. Act as a resource and mentor to other healthcare team members.
  10. Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and a commitment to excellence in patient care.
  11. Attend and actively participate in team meetings and educational opportunities.
  12. Continuously evaluate and identify opportunities for improvement in patient care delivery and workflow processes.
  13. Maintain confidentiality and adhere to ethical and legal standards in patient care.
  14. Perform other duties as assigned by the healthcare leadership team.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) From An Accredited Institution: This Qualification Is Necessary To Ensure That The Candidate Has A Solid Foundation In Nursing Principles And Practices, As Well As The Necessary Clinical Experience And Knowledge To Perform The Duties Of A Registered Nurse Care Coordinator For Ortho/Spine Patients.

  • Current And Valid Registered Nurse (Rn) License In The State Of Georgia: A Valid Rn License Is Required To Practice As A Nurse In The State Of Georgia And Is A Basic Requirement For Any Nursing Position At Emory Healthcare. This License Ensures That The Candidate Has Met The State's Educational And Competency Requirements To Provide Safe And Effective Patient Care.

  • Minimum Of 2 Years Of Clinical Experience In Ortho/Spine Nursing: This Qualification Demonstrates The Candidate's Experience And Expertise In Caring For Ortho/Spine Patients, As Well As Their Familiarity With The Unique Challenges And Complexities Of This Patient Population. It Also Ensures That The Candidate Has A Strong Understanding Of The Specific Protocols And Procedures Required For Ortho/Spine Care.

  • Certification In Orthopedic Nursing (Onc) Or Spinal Cord Injury (Sci) Nursing: While Not A Requirement, Having A Certification In Onc Or Sci Nursing Is Highly Desirable For This Role. These Certifications Indicate A Specialized Knowledge And Skill Set In Caring For Ortho/Spine Patients, And Can Help To Differentiate The Candidate From Other Applicants.

  • Strong Communication And Leadership Skills: As A Care Coordinator, The Registered Nurse Will Be Responsible For Coordinating Care Plans And Collaborating With A Multi-Disciplinary Team. Therefore, Strong Communication And Leadership Skills Are Essential To Effectively Communicate With Patients, Families, And Other Healthcare Professionals And To Ensure The Delivery Of High-Quality Care.
