- Verify and reconcile patient medication lists to ensure accuracy and up-to-date information.
- Communicate with patients, caregivers, and healthcare providers to obtain necessary medication information.
- Collaborate with pharmacy and nursing staff to resolve discrepancies and ensure safe medication administration.
- Maintain organized and detailed records of medication reconciliation process.
- Educate patients and caregivers on the importance of accurate medication lists and the reconciliation process.
- Assist in the development and implementation of medication reconciliation protocols and procedures.
- Participate in team meetings and trainings related to medication reconciliation.
- Adhere to all HIPAA and patient confidentiality guidelines.
- Monitor and report any trends or issues related to medication reconciliation.
- Provide exceptional customer service to patients and their families.
- Maintain a clean and organized work environment.
- Demonstrate a strong understanding of medications and their potential interactions.
- Perform other related duties as assigned by supervising healthcare professionals.
Bachelor's Degree In Healthcare Administration Or Related Field.
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Medication Reconciliation Or Pharmacy.
Strong Understanding Of Medical Terminology And Pharmacology.
Knowledge Of Electronic Medical Record Systems And Experience With Data Entry.
Excellent Organizational And Communication Skills, With The Ability To Work Collaboratively With Healthcare Providers And Patients.
