
Lead Case Management Associate
At Emory Healthcare, we are looking for an experienced Lead Case Management Associate to join our team and support the organization's mission of providing the highest quality of patient care. This individual will work closely with our team of Case Managers, providing guidance and leadership to ensure that our patients receive the best possible care.The ideal candidate for this position should have a background in Case Management or a related field, with a strong understanding of the health care system. They should have excellent organizational and communication skills, and be able to manage a large and complex caseload. The Lead Case Management Associate should also be comfortable working in a fast-paced environment, and be able to handle multiple tasks and prioritize workloads.We are looking for a passionate and knowledgeable individual who is committed to providing the highest level of care to our patients. Responsibilities:
• Provide guidance and leadership to Case Managers to ensure patients receive the best possible care. • Manage a large and complex caseload. • Prioritize workloads. • Maintain effective communication with healthcare team and other stakeholders. • Ensure compliance with all applicable laws, regulations, and policies. • Monitor patient outcomes and modify treatment plans accordingly. • Track and document patient progress. • Develop and implement strategies to improve the quality of care. • Analyze data to assess the effectiveness of programs and services. • Stay up to date on relevant healthcare trends and best practices. • Provide training and mentorship to new and existing staff. • Assist in resolving conflicts between patients, staff, and other stakeholders.
