  1. Coordinate and manage the care of patients undergoing heart transplant surgery.
  2. Ensure that patients receive timely and appropriate treatment before, during, and after the transplant procedure.
  3. Serve as the primary point of contact for patients, families, and healthcare providers regarding all aspects of the heart transplant process.
  4. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients.
  5. Ensure all necessary medical tests and evaluations are completed prior to transplant surgery.
  6. Educate patients and families on the transplant process, including pre- and post-operative care, medication management, and potential complications.
  7. Monitor and assess patient progress, identify any issues or concerns, and make necessary adjustments to the treatment plan.
  8. Maintain accurate and up-to-date medical records for each patient.
  9. Communicate with insurance companies and assist with obtaining necessary authorizations and approvals for transplant procedures.
  10. Participate in quality improvement initiatives to enhance patient outcomes and ensure compliance with regulatory standards.
  11. Stay current on advancements and developments in the field of heart transplantation.
  12. Provide emotional support and counseling to patients and families throughout the transplant process.
  13. Serve as a resource and mentor to other healthcare professionals and staff members.
  14. Adhere to all hospital policies and procedures, as well as ethical and legal standards.
  15. Demonstrate a commitment to the mission and values of Emory Healthcare, providing compassionate and high-quality care to all patients.

Job Qualifications
  • Bachelor's Degree In Nursing, Healthcare Administration, Or Related Field.

  • Minimum Of 3 Years Of Experience In A Cardiac Setting, With A Focus On Transplant Coordination.

  • Certification In Transplant Coordination, Such As Cctc Or Cptc, Preferred.

  • Knowledge Of Unos And Cms Guidelines And Regulations Related To Heart Transplant.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Multidisciplinary Teams And Communicate Effectively With Patients And Their Families.
