  1. Develop and implement discharge plans for patients in collaboration with healthcare team members, patients, and their families.
  2. Evaluate patients' medical history and current condition to determine appropriate level of care and necessary resources for discharge.
  3. Coordinate with insurance companies and other healthcare facilities to ensure smooth transition of care for patients.
  4. Communicate effectively with patients and their families to educate them on the discharge process and provide necessary resources.
  5. Work closely with physicians, nurses, and other healthcare professionals to ensure safe and timely discharge of patients.
  6. Maintain accurate and up-to-date records of discharge plans and follow-up appointments.
  7. Collaborate with social workers and case managers to address any psychosocial or financial concerns that may affect the discharge process.
  8. Stay current with healthcare regulations and policies related to discharge planning.
  9. Participate in interdisciplinary team meetings to discuss patient progress and discharge plans.
  10. Assist in identifying areas for improvement in the discharge process and develop strategies to address them.
  11. Provide support and guidance to patients and their families during the discharge process.
  12. Demonstrate compassion and empathy towards patients and their families during what can be a stressful and uncertain time.
  13. Maintain confidentiality of patient information and adhere to HIPAA regulations.
  14. Participate in ongoing training and professional development opportunities to stay updated on best practices in discharge planning.
  15. Collaborate with community resources to provide post-discharge support and resources for patients as needed.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or Related Healthcare Field.

  • Minimum Of 3-5 Years Experience In Discharge Planning Or Case Management In A Healthcare Setting.

  • Knowledge Of Federal And State Regulations Related To Discharge Planning, Including Medicare And Medicaid Guidelines.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Healthcare Team Members.

  • Ability To Prioritize And Manage Multiple Complex Cases, And Make Timely And Appropriate Discharge Recommendations.
