Job Description

Emory Healthcare is seeking an experienced and compassionate Clinical Transplant Coordinator to join our team. This role requires a highly organized and motivated individual with extensive experience in patient care and transplant services. The Clinical Transplant Coordinator will be responsible for providing individualized care to patients undergoing organ transplantation, while also working closely with the transplant team to ensure a successful patient experience.The ideal candidate will possess a minimum of a bachelor’s degree in nursing, along with a current RN licensure. They should have a minimum of two years of clinical experience in transplant services, as well as experience in patient education, case management, and utilization review. Exceptional communication, collaboration, and problem-solving skills are essential to this role. Above all, the selected candidate will be an empathic advocate for our patients and their families, and have a deep commitment to providing quality and compassionate care.

Responsibilities:

  1. Provide individualized care to patients undergoing organ transplantation.
  2. Work collaboratively with the transplant team to ensure successful patient experience.
  3. Maintain up-to-date RN licensure and a minimum of two years of clinical experience in transplant services.
  4. Educate patients and their families on transplantation procedures and post-transplant care.
  5. Collaborate with other healthcare professionals to ensure quality care.
  6. Develop case management plans and coordinate utilization review.
  7. Utilize exceptional communication, collaboration, and problem-solving skills.
  8. Advocate for patient rights and provide compassionate care.
