
Clinical Pharmacist
We are seeking an experienced and passionate Clinical Pharmacist to join our outstanding team at Emory Healthcare. As a Clinical Pharmacist, you will provide safe and effective medication therapy management and clinical services to our patients and collaborate with the multidisciplinary healthcare team to ensure optimal patient care. The ideal candidate will possess a Doctor of Pharmacy degree, be licensed in the state of Georgia, have excellent interpersonal and communication skills, and be comfortable working in a fast-paced environment. Responsibilities:
- Assist in providing safe and effective medication therapy management and clinical services to patients.
- Collaborate with the multidisciplinary healthcare team to ensure optimal patient care.
- Maintain current knowledge of pharmaceuticals and evidence-based clinical practice guidelines.
- Monitor patient progress and identify any potential medication-related issues or opportunities for improvement.
- Perform drug utilization review, medication reconciliation, and medication therapy management for individual patients.
- Provide patient counseling and education about the use of medications and potential side effects.
- Maintain accurate and up-to-date patient medication profiles.
- Participate in the development and implementation of clinical pharmacy services and protocols.
- Participate in pharmacy and hospital committees, educational programs, and research activities.
- Comply with all legal and regulatory requirements pertaining to the practice of pharmacy.
