- Review and analyze patient medical records to ensure accuracy, completeness, and compliance with coding and billing regulations.
- Collaborate with healthcare providers to obtain missing or incomplete information in patient records.
- Identify and report documentation deficiencies to the appropriate parties for prompt resolution.
- Conduct regular audits of medical records and coding to identify potential areas for improvement.
- Educate and train healthcare providers on proper clinical documentation practices.
- Stay up-to-date on coding and billing regulations and guidelines to ensure compliance.
- Serve as a liaison between healthcare providers, coding and billing teams, and insurance companies to resolve documentation-related issues.
- Work with coding and billing teams to ensure accurate and timely reimbursement for services rendered.
- Maintain confidentiality and security of patient records and information.
- Communicate effectively with team members and other healthcare professionals to ensure coordinated and comprehensive patient care.
- Participate in quality improvement initiatives to enhance the accuracy and completeness of clinical documentation.
- Adhere to all hospital policies, procedures, and standards of practice.
- Provide excellent customer service to patients, families, and other healthcare professionals.
- Complete all required documentation and reports accurately and on time.
- Contribute to a positive and supportive work environment that fosters teamwork and collaboration.
Certified Clinical Documentation Specialist (Ccds) Or Certified Documentation Improvement Practitioner (Cdip) Certification.
Bachelor's Degree In Nursing Or A Healthcare-Related Field.
Minimum Of 2 Years Experience In Clinical Documentation Improvement, Preferably In An Acute Care Setting.
Strong Knowledge Of Icd-10-Cm/Pcs Coding And Drg Assignment.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Providers, Coding Staff, And Other Members Of The Healthcare Team.
