Job Description

Elevance Health is seeking a highly motivated, service-oriented Telephonic Nurse Case Manager to join our dynamic team. The ideal candidate will have an unwavering commitment to providing the best patient care and experience. You will be responsible for proactively managing the needs of the patient across the continuum of care, and working collaboratively with other healthcare providers. To be successful in this role, you must possess a strong attention to detail, excellent communication and interpersonal skills, and a deep knowledge of medical terminology and the healthcare industry. The successful candidate will be required to have a current RN license, as well as a minimum of three years of experience in case management, population health, or utilization management. Knowledge of case management software and EMR systems, as well as experience with managed care or payer based care, is preferred. We are looking for a team player who is passionate about providing excellent service to our patients and clients. Responsibilities:

  • Monitor patient care across the continuum of healthcare services
  • Communicate and collaborate with other healthcare providers
  • Utilize case management software and EMR systems
  • Adhere to managed care and payer policies
  • Provide exceptional customer service to patients and clients
  • Maintain a professional and knowledgeable attitude with patients and families
  • Utilize medical terminology and clinical knowledge to assess patient needs
  • Follow up with patients post discharge
  • Conduct utilization review and address any gaps in service
  • Monitor patient progress and document all care and services provided to patients
  • Monitor patient outcomes and escalate any issues to the appropriate team
