Referral and Resource Specialist

At Elevance Health, we’re looking for a Referral and Resource Specialist who will be responsible for providing referrals and resources to individuals with intellectual and developmental disabilities and their families. We’re looking for an individual with a Bachelor’s degree in a human services related field, at least three years of experience working with individuals with disabilities, and an understanding of community based and entitlement programs. 1. Provide referrals and resources to individuals with intellectual and developmental disabilities and their families.
2. Ensure quality service is provided to clients.
3. Provide client-centered, holistic care.
4. Utilize a Bachelor’s degree in a human services related field.
5. Have at least three years of experience working with individuals with disabilities.
6. Understand community based and entitlement programs.
7. Possess knowledge of the local community.
8. Understand the principles of independent living and family support.
9. Participate in meetings with teams, families, and other providers as needed.
10. Keep accurate records of services provided and client progress.
11. Follow applicable laws, regulations, and ethical standards.
