Referral and Resource Specialist

Responsibilities:

1. Develop and manage a comprehensive referral process, ensuring that individuals are connected to the appropriate services and resources
2. Locate and research resources, including government, community, and private organizations, to meet individual needs
3. Meet with individuals to assess needs and provide referrals to appropriate services and resources
4. Utilize electronic health records to track and document referral and resource information
5. Maintain up-to-date knowledge of available resources and services
6. Facilitate communication between service providers and individuals
7. Provide follow-up assistance as needed
8. Collaborate with other departments and organizations to coordinate patient care
9. Ensure compliance with all relevant laws and regulations
10. Prepare reports and other documents as needed
