Nurse Disease Manager

Additionally, the Nurse Disease Manager must have experience in disease management, case management, or a related field. Responsibilities:

1. Provide outstanding healthcare services to patients.
2. Utilize excellent analytical and problem-solving skills to coordinate care for patients.
3. Develop and maintain relationships with patients, families, and other health care providers.
4. Monitor patient health, develop care plans, and provide support to patients and families.
5. Document patient care activities and progress in medical records.
6. Monitor and evaluate patient outcomes.
7. Participate in quality improvement activities.
8. Remain current on new developments and best practices in disease management, case management, and related fields.
9. Comply with applicable laws, regulations, and policies.
