Job Description

We are seeking a compassionate and dedicated Nurse Case Manager to join our growing team at Elevance Health.   11. Serve as a resource and mentor for other healthcare team members.
  12. Maintain a high level of professionalism and confidentiality in all interactions with patients and colleagues.
  13. Participate in meetings and conferences to discuss patient cases and collaborate on treatment plans.
  14. Continuously strive to improve the quality of patient care and outcomes.

Job Qualifications
  • Bachelor's Degree In Nursing: A Nurse Case Manager I At Elevance Health Should Have A Minimum Of A Bachelor's Degree In Nursing From An Accredited University. This Education Provides The Necessary Foundation In Healthcare And Nursing Practices.

  • Active Nursing License: A Valid And Active Nursing License Is A Must For This Position. The Candidate Should Have A Current License In The State Where Elevance Health Operates.

  • Previous Experience In Case Management: Prior Experience In Case Management Or A Related Field Is Preferred For This Role. This Could Include Experience As A Nurse Case Manager, Utilization Review Nurse, Or Care Coordinator.

  • Knowledge Of Healthcare Regulations: A Nurse Case Manager I Should Have A Strong Understanding Of Healthcare Regulations And Compliance Standards. This Includes Knowledge Of Medicare, Medicaid, And Other Insurance Regulations.

  • Strong Communication And Organizational Skills: Effective Communication And Organizational Skills Are Essential For A Successful Nurse Case Manager I. They Will Be Responsible For Coordinating Care With Patients, Families, And Other Healthcare Professionals, As Well As Managing And Organizing A Large Caseload.
