  3. Work closely with healthcare providers to ensure proper implementation of care plans and monitor patient progress.
  4. Serve as a liaison between patients, their families, and healthcare teams to facilitate effective communication and promote patient-centered care.
  5. Conduct regular assessments of patient care plans and make necessary adjustments as needed.
  6. Monitor and document patient progress and communicate any changes or concerns to the healthcare team.
  7. Collaborate with insurance companies to ensure proper coverage for patients' medical needs.
  8. Educate patients and their families on their health conditions, treatment plans, and resources available to them.
  9. Maintain accurate and up-to-date patient records and documentation.
  10. Keep abreast of current healthcare practices and regulations to ensure compliance and provide the best possible care.
  11. Participate in team meetings and contribute to the development of policies and procedures for patient care.
  12. Uphold company values and promote a positive work environment.
  13. Continuously strive to improve the quality of care and patient satisfaction.
  14. Adhere to ethical and professional standards in all interactions with patients, colleagues, and other healthcare professionals.

Job Qualifications
  • Bachelor's Degree In Nursing: A Nurse Case Manager At Elevance Health Should Possess A Bachelor's Degree In Nursing From An Accredited Institution. This Educational Background Ensures A Strong Foundation In Healthcare Principles And Practices.

  • Active Rn License: A Nurse Case Manager Must Hold An Active Registered Nurse (Rn) License In The State Where They Will Be Practicing. This License Ensures That The Individual Has Met The Necessary Requirements To Provide Safe And Effective Patient Care.

  • Case Management Certification: Elevance Health May Prefer Or Require A Nurse Case Manager To Have A Certification In Case Management, Such As The Certified Case Manager (Ccm) Or Certified Professional In Healthcare Management (Cphm). This Certification Demonstrates A High Level Of Knowledge And Expertise In Case Management.

  • Clinical Experience: The Ideal Candidate For A Nurse Case Manager At Elevance Health Should Have A Minimum Of 2-3 Years Of Clinical Experience As A Registered Nurse. This Experience Provides A Solid Understanding Of Patient Care, Treatment Plans, And Healthcare Systems.

  • Strong Communication And Interpersonal Skills: As A Nurse Case Manager, Effective Communication And Interpersonal Skills Are Essential For Collaborating With Patients, Families, Physicians, And Other Healthcare Professionals.
