- Develop and implement effective strategies for the coordination and delivery of long-term services and supports for members.
- Oversee and manage a team of service coordinators, providing guidance and support to ensure high-quality and cost-effective care for members.
- Collaborate with internal departments and external partners to ensure seamless and comprehensive care for members.
- Stay up-to-date on industry trends and best practices related to service coordination and healthcare management.
- Monitor and analyze data to identify areas for improvement and develop action plans to address any gaps in care.
- Develop and maintain relationships with key stakeholders, including members, families, providers, and community organizations.
- Ensure compliance with all applicable regulations and standards related to service coordination and healthcare management.
- Provide leadership and mentorship to team members, fostering a positive and collaborative work environment.
- Advocate for the needs and rights of members and their families, ensuring their voices are heard and their needs are met.
- Participate in the development and implementation of company-wide policies and procedures, ensuring alignment with organizational goals and values.
- Serve as a role model for professionalism, integrity, and ethical behavior in all interactions with members, colleagues, and partners.
- Continuously evaluate and improve processes and workflows to enhance the efficiency and effectiveness of service coordination activities.
- Represent the company at conferences, meetings, and other events to promote our mission and services.
- Prepare and present reports to senior management, providing updates on key metrics, initiatives, and challenges.
- Collaborate with HR to recruit, train, and retain top talent for the service coordination team.
A Minimum Of 5 Years Of Experience In Healthcare Management Or Related Field.
A Valid Rn, Lsw, Lcsw, Or Lmhc License In The State Of Employment.
Strong Knowledge And Understanding Of Ltss (Long-Term Services And Supports) Programs And Service Coordination.
Proven Experience In Managing And Leading A Team, Including Training, Coaching, And Performance Evaluation.
Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Internal And External Stakeholders, Including Healthcare Providers, Community Organizations, And Government Agencies.
