  1. Lead and manage the LTSS/Service Coordination program within Elevance Health.
  2. Oversee a team of dedicated professionals and provide guidance and support to ensure exceptional care coordination services for our members.
  3. Collaborate with other departments and teams to ensure streamlined and efficient processes for the LTSS/Service Coordination program.
  4. Develop and implement strategies to improve the quality and effectiveness of care coordination services.
  5. Conduct regular trainings and provide ongoing education for team members to enhance their skills and knowledge.
  6. Monitor and analyze program data to identify trends and make recommendations for improvements.
  7. Maintain up-to-date knowledge of industry regulations and standards related to LTSS/Service Coordination.
  8. Build and maintain positive relationships with community organizations, healthcare providers, and other stakeholders to enhance the program's reach and impact.
  9. Participate in the recruitment and hiring process for new team members.
  10. Ensure compliance with all company policies and procedures, as well as relevant laws and regulations.
  11. Set and monitor performance goals for the team and provide regular feedback and evaluations.
  12. Serve as a mentor and coach to team members, fostering a supportive and collaborative work environment.
  13. Act as a liaison between Elevance Health and other healthcare organizations, representing the company with professionalism and integrity.
  14. Continuously assess and improve the program to meet the changing needs of our members and the healthcare industry.
  15. Contribute to the overall mission and goals of Elevance Health to revolutionize healthcare for our members.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Counseling, Or Related Field.

  • Current Rn, Lsw, Lcsw, Or Lmhc License In The State Of Employment.

  • Minimum Of 3-5 Years Of Experience In Case Management Or Service Coordination.

  • Knowledge Of Ltss (Long-Term Services And Supports) Programs And Regulations.

  • Previous Experience In A Managerial Or Supervisory Role, Preferably In Healthcare Or Social Services.
