Job Description

We are currently seeking a highly organized and empathetic Foster Care Case Manager to join our team. As a Case Manager, you will play a vital role in advocating for the well-being and stability of children in the foster care system.   1. Develop and maintain strong relationships with foster children, their families, and foster care agencies.
  2. Conduct thorough assessments of each child's needs and develop individualized care plans.
  3. Advocate for the best interests of each child and ensure their safety and well-being.
  4. Collaborate with various stakeholders including social workers, therapists, and legal professionals to coordinate services for children in foster care.
  5. Monitor and document the progress and development of each child in the foster care system.
  6. Ensure compliance with all relevant foster care regulations and policies.
  7. Provide crisis intervention and support to foster children and their families as needed.
  8. Facilitate regular meetings with all involved parties to discuss the child's progress and address any concerns.
  9. Communicate effectively with the child's biological family, providing updates and addressing any issues that may arise.
  10. Monitor and maintain accurate records of all case activities, including progress notes and reports.
  11. Maintain confidentiality and professionalism when handling sensitive information.
  12. Stay up-to-date on current foster care practices and policies to ensure the best possible care for children.
  13. Participate in team meetings and trainings to enhance knowledge and skills related to foster care.
  14. Collaborate with community resources to provide additional support and services for foster children.
  15. Continuously evaluate and assess the effectiveness of the care plan and make necessary adjustments to ensure the child's needs are being met.
  16. Provide emotional support and encouragement to foster children, helping them to build resilience and self-esteem.
  17. Assist with the transition process for children entering or exiting the foster care system.
  18. Maintain a positive and professional attitude at all times, promoting a supportive and inclusive work environment.
  19. Attend court hearings and provide testimony or reports as needed.
  20. Uphold the mission and values of Elevance Health and represent the organization in a positive and professional manner at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Counseling, Psychology, Or A Related Field.

  • Minimum Of 2 Years Of Experience Working In Child Welfare Or Foster Care Services.

  • Knowledge Of State And Federal Regulations Regarding Foster Care And Child Welfare.

  • Strong Communication And Interpersonal Skills To Effectively Work With Foster Children, Foster Families, And Other Professionals.

  • Ability To Handle A High Caseload And Prioritize Tasks In A Fast-Paced Environment.
