Community Outreach Care Specialist
We are looking for a passionate and driven Community Outreach Care Specialist to join the Elevance Health team! As a Care Specialist, you will be the face of our mission to ensure that everyone in the community has access to quality healthcare. You will be responsible for connecting with community members and providing support, guidance, and resources to help them access the care they need.This position requires excellent interpersonal skills, a strong work ethic, and a passion for making a positive impact on the lives of others. Responsibilities:
- Develop and maintain relationships with community members, healthcare providers, and other stakeholders to best serve the needs of the community.
- Develop and implement plans for community outreach and engagement activities.
- Educate community members on the importance of healthcare access and the available resources.
- Provide support to individuals and families in navigating the healthcare system.
- Connect individuals with healthcare providers and resources according to their needs.
- Monitor progress of individuals accessing healthcare services and resources.
- Coordinate with external organizations to ensure access to quality healthcare.
- Act as a liaison between the community and healthcare providers.
- Advocate for healthcare policies that support the needs of the community.
- Develop reports and presentations to track activities and outcomes.
