Behavioral Health Case Manager

Responsibilities:

• Provide excellent care to patients in a behavioral health setting

• Effective communication with clients and other members of the multidisciplinary team

• Facilitate meetings to discuss patient progress and treatment plans

• Develop and maintain relationships with clients and other stakeholders

• Maintain accurate and up-to-date case notes and records

• Monitor and evaluate client progress and provide feedback

• Ensure compliance with all relevant regulations and policies

• Utilize available resources to effectively manage cases

• Provide crisis intervention and emergency assistance as needed
