Job Description

We are looking for a highly-skilled and compassionate Behavioral Health Case Manager to join our team. The successful candidate will be a positive, patient-centered individual who is committed to helping our clients achieve their goals.The Behavioral Health Case Manager will be responsible for providing comprehensive case management and patient/family education services. They will collaborate with other departments to ensure that our clients receive timely and quality care. This position requires excellent interpersonal skills, as well as the ability to assess, plan, coordinate, monitor, and evaluate services.To be considered for this role, applicants must have a Bachelor’s degree in behavioral health or a related field and a minimum of two years of experience in a social work or case management setting. A current state-level licensure is preferred. Additionally, the ideal candidate will possess strong interpersonal skills and a deep commitment to helping those in need.

Responsibilities:

  1. Provide comprehensive case management and patient/family education services.
  2. Collaborate with other departments to ensure that clients receive timely and quality care.
  3. Assess, plan, coordinate, monitor, and evaluate services.
  4. Maintain a positive, patient-centered attitude.
  5. Utilize excellent interpersonal skills to interact with clients.
  6. Maintain a current state-level licensure.
  7. Document patient care and progress in medical records.
  8. Utilize evidence-based practice to ensure quality care.
  9. Develop treatment plans that meet individual needs.
  10. Provide crisis intervention services as needed.
