Behavioral Health Care Manager

Responsibilities:

1. Develop and implement individualized patient care plans to address mental health needs
2. Monitor patients progress and adjust care plans as needed
3. Provide counseling and psychotherapy services to patients
4. Collaborate with other healthcare providers to ensure effective patient care
5. Establish and maintain professional relationships with patients and their families
6. Educate patients and their families about mental health issues and treatment options
7. Ensure patient compliance with treatment plans
8. Document patient data and progress in electronic health records
9. Serve as a resource for patients and their families
10. Participate in case reviews and conferences to discuss treatment plans
11. Manage referrals to other healthcare professionals
12. Perform crisis intervention when needed
