Social Worker

This position requires excellent interpersonal and communication skills, a passion for providing social services to individuals and families, and the ability to work within a multidisciplinary health care team. The Social Worker will be responsible for conducting individual and family assessments, providing individual and family counseling services, and developing and implementing treatment plans. Responsibilities:

1. Conduct individual and family assessments to identify needs and develop treatment plans.
2. Provide individual and family counseling services to support patients.
3. Collaborate with multidisciplinary health care team to ensure effective and comprehensive care.
4. Develop and implement treatment plans based on assessment and counseling services.
5. Maintain current license in the State of North Carolina.
6. Utilize problem-solving and interpersonal skills to provide compassionate care.
7. Remain up-to-date on social services resources and best practices.
8. Track patient progress and document results in the patient’s electronic medical record.
