  1. Act as a resource and point of contact for patients and their families regarding population health initiatives and resources.
  2. Implement and coordinate population health programs and interventions, including patient outreach and education.
  3. Collaborate with healthcare providers and other team members to develop and implement strategies for improving patient health outcomes.
  4. Develop and maintain relationships with community organizations and resources to support population health efforts.
  5. Conduct assessments of patient needs and barriers to care, and provide appropriate referrals and resources.
  6. Monitor and track patient progress and outcomes in population health programs.
  7. Maintain accurate and up-to-date documentation of patient interactions and interventions.
  8. Participate in training and educational opportunities to further develop knowledge of population health principles and strategies.
  9. Communicate effectively with patients, families, and healthcare team members to promote a patient-centered approach to care.
  10. Adhere to all relevant policies, procedures, and regulations related to population health initiatives.
  11. Serve as a positive and professional representative of Duke University Health System.
  12. Continuously seek opportunities for improvement and innovation in population health initiatives.
  13. Maintain confidentiality and handle sensitive patient information with utmost discretion.
  14. Support and promote the overall mission and values of Duke University Health System.
  15. Other duties as assigned by supervisor.

Job Qualifications
  • Bachelor's Degree In Public Health, Healthcare Administration, Or A Related Field.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Patient Engagement Or Population Health.

  • Knowledge Of Population Health Principles And Strategies For Improving Health Outcomes.

  • Strong Communication And Interpersonal Skills, With The Ability To Engage And Motivate Patients.

  • Proficiency In Data Analysis And Reporting, With Experience Using Electronic Medical Records And Population Health Software.
