- Greet and welcome patients and visitors to Duke University Health System in a friendly and professional manner.
- Provide essential resources and services to support population health initiatives, including but not limited to health education, community outreach, and care coordination.
- Collaborate with other members of the population health team to ensure seamless delivery of services and resources.
- Assist patients in navigating the healthcare system and connecting them with appropriate resources and services.
- Maintain accurate and detailed records of patient interactions and services provided.
- Continuously monitor and evaluate the effectiveness of population health programs and make recommendations for improvement.
- Stay up-to-date on current healthcare trends, community resources, and best practices in population health.
- Serve as a liaison between patients and healthcare providers, advocating for the needs and well-being of the community.
- Develop and maintain strong relationships with community organizations and partners to expand and enhance resources available to patients.
- Provide excellent customer service to patients and their families, addressing any concerns or issues in a timely and compassionate manner.
- Participate in training and education programs to enhance knowledge and skills related to population health.
- Adhere to all organizational policies, procedures, and regulations to ensure compliance with healthcare standards.
- Contribute to the overall success of the population health team by actively participating in team meetings and providing valuable insights and ideas.
- Maintain strict confidentiality of patient information and adhere to all HIPAA regulations.
- Continuously strive to improve the quality and efficiency of services provided to patients and the community.
Bachelor's Degree In A Related Field Such As Public Health, Healthcare Administration, Or Health Sciences.
Experience In Data Analysis And Interpretation, Preferably In A Healthcare Setting.
Knowledge Of Population Health Principles And Strategies, Including Community Health Needs Assessment And Health Disparities.
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Diverse Stakeholders Such As Healthcare Providers, Community Organizations, And Government Agencies.
Strong Organizational Skills And Ability To Multitask In A Fast-Paced Environment, While Maintaining Attention To Detail And Meeting Deadlines.
