Licensed Practical Nurse - Home Health

Responsibilities:
- Provide high quality nursing care in the home setting.
- Collaborate with other members of the healthcare team to provide comprehensive care to patients and families.
- Maintain a professional attitude and demonstrate excellent communication skills.
- Deliver quality care in a home setting.
- Ensure compliance with all applicable regulations and laws pertaining to the practice of nursing.
- Develop and implement patient care plans.
- Monitor patient health and document changes in patient condition.
- Administer medications and treatments as prescribed by a physician.
- Perform assessments to identify and address patient needs.
- Provide patient and family education regarding health care needs and self-care.
Current Cpr/Bls Certification
