
Clinical Pharmacist Rheumatology and Dermatology

- Conduct comprehensive medication reviews and assessments for patients with rheumatological and dermatological conditions, ensuring safe and effective medication therapy.
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized treatment plans for patients.
- Monitor patient response to medication therapy and make necessary adjustments to optimize outcomes.
- Provide patient education on medication use, side effects, and potential interactions.
- Stay current on the latest research and developments in rheumatology and dermatology to inform treatment decisions.
- Participate in medication reconciliation processes to ensure accurate and up-to-date medication records.
- Maintain accurate and complete patient medication profiles.
- Comply with all regulatory and safety standards in the handling and dispensing of medications.
- Contribute to quality improvement initiatives and participate in departmental meetings and committees.
- Serve as a resource to other healthcare professionals regarding medication therapy and drug information.
- Maintain a professional and collaborative working relationship with all members of the healthcare team.
- Participate in continuing education and professional development activities to enhance knowledge and skills.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Residency Training In Clinical Pharmacy Practice Or A Specialized Residency In Rheumatology Or Dermatology.
Board Certification In Pharmacotherapy By The Board Of Pharmacy Specialties (Bps).
Experience Working In A Clinical Setting, Preferably In A Rheumatology Or Dermatology Specialty.
Strong Knowledge Of Medications Used In The Treatment Of Rheumatologic And Dermatologic Conditions, As Well As Experience In Managing Complex Medication Regimens.
