We are currently seeking a highly skilled and dedicated Clinical Pharmacist to join our Rheumatology and Dermatology team. This role will play a crucial part in optimizing medication therapy for patients with rheumatological and dermatological conditions, collaborating with a multidisciplinary team to provide comprehensive care.   1. Conduct comprehensive medication reviews and assessments for patients with rheumatological and dermatological conditions, ensuring safe and effective medication therapy.
  2. Collaborate with physicians, nurses, and other healthcare professionals to develop individualized treatment plans for patients.
  3. Monitor patient response to medication therapy and make necessary adjustments to optimize outcomes.
  4. Provide patient education on medication use, side effects, and potential interactions.
  5. Stay current on the latest research and developments in rheumatology and dermatology to inform treatment decisions.
  6. Participate in medication reconciliation processes to ensure accurate and up-to-date medication records.
  7. Maintain accurate and complete patient medication profiles.
  8. Comply with all regulatory and safety standards in the handling and dispensing of medications.
  9. Contribute to quality improvement initiatives and participate in departmental meetings and committees.
  10. Serve as a resource to other healthcare professionals regarding medication therapy and drug information.
  11. Maintain a professional and collaborative working relationship with all members of the healthcare team.
  12. Participate in continuing education and professional development activities to enhance knowledge and skills.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.

  • Residency Training In Clinical Pharmacy Practice Or A Specialized Residency In Rheumatology Or Dermatology.

  • Board Certification In Pharmacotherapy By The Board Of Pharmacy Specialties (Bps).

  • Experience Working In A Clinical Setting, Preferably In A Rheumatology Or Dermatology Specialty.

  • Strong Knowledge Of Medications Used In The Treatment Of Rheumatologic And Dermatologic Conditions, As Well As Experience In Managing Complex Medication Regimens.
