- Ensure safe and effective use of medication: The primary responsibility of the Clinical Pharmacist will be to ensure the safe and effective use of medications for our inpatient population. This includes reviewing medication orders, monitoring for potential drug interactions, and providing appropriate dosing and administration recommendations.
- Collaborate with healthcare team: The Clinical Pharmacist will work closely with physicians, nurses, and other healthcare professionals to provide comprehensive pharmaceutical care to patients. They will serve as a medication therapy expert and consult with the healthcare team on appropriate drug therapy options.
- Conduct medication reconciliation: The Clinical Pharmacist will be responsible for conducting medication reconciliation for all patients upon admission and discharge. This will involve comparing medication lists, identifying discrepancies, and making appropriate recommendations to ensure continuity of care.
- Monitor and manage medication therapy: The Clinical Pharmacist will be responsible for monitoring and managing medication therapy for inpatients. This includes reviewing medication regimens, identifying and resolving any drug-related problems, and adjusting therapy as needed.
- Provide patient education: The Clinical Pharmacist will play a key role in educating patients and their families about their medications. This may include explaining potential side effects, proper administration techniques, and the importance of adherence to medication therapy.
- Participate in quality improvement initiatives: As a member of our team, the Clinical Pharmacist will be expected to actively participate in quality improvement initiatives to enhance patient outcomes. This may involve evaluating medication processes and making recommendations for improvement.
- Stay up-to-date with current practice standards: The Clinical Pharmacist will be responsible for staying current with the latest developments and best practices in pharmacy. This may include attending continuing education programs, participating in professional organizations, and staying informed about new medications and treatment guidelines.
- Adhere to policies and regulations: The Clinical Pharmacist will be expected to adhere to all policies, procedures, and regulations set forth by Duke University Health System, as well as state and federal laws governing the practice of pharmacy.
- Maintain accurate records:
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Current And Valid State License To Practice Pharmacy.
Minimum Of 2 Years Of Experience In An Inpatient Pharmacy Setting.
Experience With Electronic Health Records And Medication Management Systems.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Healthcare Teams And Patients.
