
Clinical Pharmacist - Inpatient Operations

- Collaborate with physicians, nurses, and other healthcare providers to review and optimize medication therapy for inpatient population.
- Conduct patient medication histories and medication reconciliations to ensure safe and effective treatment plans.
- Monitor patient response to medication therapy and adjust as necessary.
- Participate in interdisciplinary rounds and provide drug information to healthcare team.
- Review and interpret laboratory data to identify potential drug interactions or adverse effects.
- Utilize clinical knowledge and evidence-based guidelines to make recommendations for medication therapy.
- Ensure compliance with all relevant laws, regulations, and policies related to medication use.
- Maintain accurate and complete medication records for patients.
- Educate patients and caregivers on medication therapy, including proper administration, potential side effects, and drug interactions.
- Participate in quality improvement initiatives to promote safe and effective medication use.
- Maintain current knowledge of new drugs, dosage forms, and treatment protocols.
- Serve as a resource for drug information and provide guidance to other healthcare professionals.
- Communicate effectively with patients, families, and healthcare team members to promote understanding and adherence to medication therapy.
- Adhere to all safety protocols and standards to prevent medication errors.
- Continuously evaluate and improve medication use processes to optimize patient outcomes.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Current Licensure As A Pharmacist In The State Of North Carolina
Minimum Of 2 Years Of Experience In A Clinical Pharmacy Role, Preferably In An Inpatient Setting
Board Certification In Pharmacotherapy (Bcps) Or Other Relevant Specialty (E.g. Critical Care, Oncology)
Strong Knowledge Of Inpatient Pharmacy Operations, Including Medication Dispensing And Clinical Decision-Making Processes
