1. Collaborate with physicians, nurses, and other healthcare providers to review and optimize medication therapy for inpatient population.
2. Conduct patient medication histories and medication reconciliations to ensure safe and effective treatment plans.
3. Monitor patient response to medication therapy and adjust as necessary.
4. Participate in interdisciplinary rounds and provide drug information to healthcare team.
5. Review and interpret laboratory data to identify potential drug interactions or adverse effects.
6. Utilize clinical knowledge and evidence-based guidelines to make recommendations for medication therapy.
7. Ensure compliance with all relevant laws, regulations, and policies related to medication use.
8. Maintain accurate and complete medication records for patients.
9. Educate patients and caregivers on medication therapy, including proper administration, potential side effects, and drug interactions.
10. Participate in quality improvement initiatives to promote safe and effective medication use.
11. Maintain current knowledge of new drugs, dosage forms, and treatment protocols.
12. Serve as a resource for drug information and provide guidance to other healthcare professionals.
