- Provide expert clinical pharmacy services to patients with infectious diseases in the outpatient clinic setting.
- Collaborate with physicians and other healthcare providers to develop and implement effective treatment plans for patients.
- Conduct medication reviews and make recommendations for appropriate therapy adjustments based on patient response and laboratory values.
- Serve as a resource and consultant for healthcare providers on the appropriate use of antimicrobial agents and other medications used in the treatment of infectious diseases.
- Monitor and manage medication therapy for patients, including dosing adjustments, drug interactions, and adverse effects.
- Participate in patient education and counseling on appropriate use of medications and adherence to treatment plans.
- Contribute to the development and implementation of research initiatives related to infectious diseases.
- Stay current with new developments and advancements in the field of infectious diseases and incorporate this knowledge into practice.
- Maintain accurate and thorough patient records, including medication histories, progress notes, and treatment plans.
- Collaborate with other members of the healthcare team to ensure optimal patient outcomes and promote a multidisciplinary approach to patient care.
- Adhere to all regulatory and safety standards, including maintaining proper storage and handling of medications.
- Act as a preceptor and mentor to pharmacy students and residents, as well as provide ongoing education and training to pharmacy staff.
- Contribute to quality improvement initiatives and participate in departmental meetings and committees.
- Maintain professional competency and licensure through continuing education and participation in professional organizations.
- Demonstrate a commitment to patient-centered care and a dedication to making a positive impact in the field of infectious diseases.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School.
Completion Of A Clinical Pharmacy Residency Program, Preferably With A Focus On Infectious Diseases.
Board Certification In Pharmacotherapy Or Infectious Diseases From The Board Of Pharmacy Specialties.
Experience Working In An Outpatient Infectious Diseases Clinic Or Similar Setting.
Strong Understanding Of Infectious Diseases And The Ability To Develop And Implement Treatment Plans Based On Patient-Specific Factors And Antimicrobial Stewardship Principles.
