  1. Conduct comprehensive medication reviews to ensure safe and effective use of medication for patients.
  2. Collaborate with other healthcare professionals to develop and implement patient-specific medication plans.
  3. Monitor patients' medication therapies and make appropriate adjustments as needed.
  4. Provide drug information and clinical support to healthcare professionals and patients.
  5. Participate in medication reconciliation processes to ensure accurate and up-to-date medication records.
  6. Develop and maintain a thorough understanding of current pharmaceutical products, trends, and developments.
  7. Comply with all state and federal regulations, as well as Duke University Health System policies and procedures.
  8. Educate patients on proper medication use, potential side effects, and drug interactions.
  9. Conduct drug utilization reviews and identify opportunities for cost-saving measures.
  10. Participate in quality improvement initiatives and contribute to the development of evidence-based medication guidelines.
  11. Maintain accurate and timely documentation of all medication-related activities.
  12. Stay informed about new medications and therapies through continuing education and professional development.
  13. Aid in the training and supervision of pharmacy technicians and interns.
  14. Provide excellent customer service to patients, families, and other healthcare professionals.
  15. Act as a resource for drug information and provide guidance to healthcare professionals on medication-related issues.
  16. Adhere to ethical and professional standards in all aspects of job performance.
  17. Maintain a clean and organized work environment.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: A Clinical Pharmacist At Duke University Health System Must Possess A Pharmd Degree From A Recognized And Accredited University. This Degree Provides A Strong Foundation In Pharmaceutical Sciences, Pharmacotherapy, And Clinical Patient Care.

  • State Licensure And Board Certification: Candidates Must Be Licensed To Practice Pharmacy In The State Of North Carolina And Hold A Board Certification In Pharmacotherapy By The Board Of Pharmacy Specialties (Bps). This Ensures That The Candidate Has The Necessary Knowledge, Skills, And Experience To Provide Effective Clinical Pharmacy Services.

  • Residency Or Fellowship Training: A Clinical Pharmacist At Duke University Health System Must Have Completed A Post-Graduate Residency Or Fellowship Program In Clinical Pharmacy Practice. This Training Provides Hands-On Experience And Specialized Knowledge In Areas Such As Internal Medicine, Pediatrics, Critical Care, Or Ambulatory Care.

  • Strong Clinical Knowledge And Experience: Candidates Must Have A Strong Clinical Background And Experience In Providing Pharmaceutical Care In An Acute Care Setting. This Includes Proficiency In Medication Therapy Management, Drug Information, And Therapeutic Monitoring.
