Job Description

In this role, you will work with a multidisciplinary team to provide comprehensive care and education to patients with diabetes.   1. Conduct comprehensive patient assessments to determine the individual needs and treatment plans for patients with diabetes.
  2. Collaborate with a multidisciplinary team to provide coordinated and holistic care to patients.
  3. Develop and implement personalized care plans for patients with diabetes, including medication management, lifestyle modifications, and education on self-management techniques.
  4. Educate patients on the importance of monitoring blood sugar levels, recognizing symptoms of hyperglycemia and hypoglycemia, and proper administration of insulin and other medications.
  5. Stay up-to-date on the latest advancements in diabetes management and participate in ongoing professional development opportunities.
  6. Monitor and evaluate patient progress, making necessary adjustments to treatment plans as needed.
  7. Provide support and guidance to patients and their families, addressing any concerns or questions related to diabetes management.
  8. Advocate for patients and collaborate with community resources to ensure access to necessary care and resources.
  9. Maintain accurate and thorough documentation of patient care and progress.
  10. Adhere to all relevant healthcare regulations and policies, including HIPAA and infection control protocols.
  11. Participate in quality improvement initiatives to enhance the overall care provided to patients with diabetes.

Job Qualifications
  • A Minimum Of 2 Years Of Experience As A Registered Nurse Or Nurse Practitioner Specialized In Endocrinology And Diabetes Management.

  • Bachelor's Degree In Nursing Or A Related Field, With A Preference For A Master's Degree In Nursing Or Nurse Practitioner Certification.

  • In-Depth Knowledge Of Diabetes Management, Including Experience With Continuous Glucose Monitoring, Insulin Pump Therapy, And Medication Management.

  • Strong Communication And Patient Education Skills, With The Ability To Effectively Explain Complex Medical Information To Patients And Their Families.

  • Current Licensure As A Registered Nurse Or Nurse Practitioner In The State Of Indiana, With Additional Certification In Endocrinology And Diabetes Management Preferred.
