Social Services Designee

The Social Services Designee will provide comprehensive social services to patients, families, and staff in the Health System.The ideal candidate will have a Bachelor’s degree in Social Work and a minimum of two years of experience in a health care setting. The successful candidate will be a compassionate and dedicated individual, capable of providing excellent patient care and advocacy. Responsibilities:

1. Provide comprehensive social services to patients, families, and staff in the Health System.
2. Coordinate services and resources for patients, and their families, in order to meet their needs.
3. Maintain confidentiality of patient information in accordance with HIPAA regulations.
4. Develop and implement individualized patient care plans.
5. Monitor patient progress and adjust care plans as needed.
6. Collaborate with other healthcare professionals to ensure best outcomes for patients.
7. Educate patients and their families on available resources and services.
8. Provide emotional and psychological support to patients and their families.
9. Advocate for patients and their families when needed.
10. Maintain updated records of all patient care.
